The Different Types of FTM Top Surgery at the CREO Clinic

There are several different surgical techniques that can be employed depending on chest, size, skin elasticity and your personal aesthetic goals. 

If you have a lot of fatty tissue around the chest, liposuction can also be used as part of top surgery to further contour the area for a masculine appearance. 

Double Incision Top Surgery

Double incision top surgery is best suited to patients with medium to large chests, who have less skin elasticity. During this procedure, two horizontal incisions are made across the chest, removing breast tissue and excess skin. The nipples are also typically resized and repositioned as free nipple grafts to suit the shape of the newly masculinised chest.

Periareolar Top Surgery

Circumareolar, or periareolar top surgery is ideal for patients with small to medium chests and good skin elasticity. During surgery, a circular incision is made around the areola, through which the breast tissue is removed and the skin is tightened. 

Keyhole Top Surgery

Keyhole top surgery is best for those with small chests, usually up to an A cup or smaller, and very good skin elasticity. This involves small incisions along the lower border of the areola to remove the breast tissue. During keyhole top surgery, the nipple usually remains attached, and little skin is removed.

It is possible to access top surgery via the NHS. However, there are very long wait lists, leading to many patients seeking private treatment. 

How much does FTM top surgery cost in the UK?

The price of top surgery can vary depending on the surgical complexity of the procedure, meaning that a full cost breakdown will only be available following a consultation with Dr. Tillo. However, in general, prices for top surgery at CREO Clinic start from £9,900.

What age is best for top surgery?

At CREO, top surgery can only be accessed over the age of 18. It is best to have top surgery when you feel that you are ready to undergo the procedure and recovery, and have had a psychological assessment for gender incongruence. 

What’s the recovery process like after top surgery?

After top surgery, you may experience side effects such as swelling, bruising and soreness around the chest which will gradually dissipate as your body heals. 

You should be able to return to light activity and work from home one week after surgery. After three weeks, you should be able to return to the office, driving and using public transport. However, you will need to wait 6 weeks before engaging in more strenuous activity. 

Will I need hormone therapy before having top surgery?

Hormone therapy is not necessary in order to have top surgery. 

What is your cut off in terms of BMI when it comes to performing top surgery?

There is no exact BMI cut off to access top surgery. However, it is generally recommended that you are at your ideal weight to get the most out of surgery and reduce the risk of complications. 

Will I have to pause my hormones in preparation for surgery?

No, you will not need to stop taking your hormones for the purposes of gender affirming care prior to surgery.  

Does FTM top surgery eliminate the risk of breast cancer?

FTM top surgery does not eliminate the risk of breast cancer. For this reason, it’s important that you still regularly check yourself for any abnormalities and speak to your doctor should you have any concerns.
